I was part of a Halo 3 Clan a few years ago. I'm not sure how other Clans function but here is how ours worked:
We broke it down my ranks, each with some form of rules.
Myself and I were the creators and leaders so we had the end all authority.
Six of our other best friends were in the next level down.
There were many, many other players underneath us.
Tryouts were held between higher ranking members and whoever was wanting to join.
Any changes or moderations of the existing rules were brought to the attention of the eight highest level members for a vote. If it passed it would go into effect.
Our clan lasted for about two years before it stopped being fun for any of us.
Each clan has its own structure so I would suggest forming some of your own guidelines and discuss them with your other clan members.
