It could be physical and mental.
Mine is:
I'm asleep, nice dream and all that. Then in my dream, everything goes black. I can't breath. And when I think all hope is lost, I wake up. My heads face down on my pillow.
Whats even worst is that I get stuck in my "laying face down" position.

Mine is easy, I had a nose job (microbriderturbinectomy) to straighten my deviated septum nearly a year ago.
Waking up and having my nose plugged with gauze while the bone heals hurt like hell for the following reasons.
A) Most obv the bone in your nose is broken and is trying to heal and almost every facial action you make causes some movement around your nose and when the morphine wears off...OOOWWWWW
B) As your nose is blocked you are unable to eat and drink properly (try swallowing while you hold your nose closed and you will see what I mean) so having to choke down food with your mouth open is painfull as you get no satisfaction from eating whatsoever and just feel sick
C) Sleeping is a nightmare as you need to sleep with your mouth open and cant really so you suffer total exhasution and sleep deprivation in attition to not being able to eat and being in immense pain...oh and talking to anyone is impossible without gasping for breath every other word.
D) Finally when they remove the gauze which is the length of a tampon from each nostril not only agrivates the broken bones but acts like a suction effect on your nose and is similar to waterboarding (dry drowning) and causes you to choke again.
All in all those few weeks were highly painfull in many ways and wholly unpleasant

Worse pain I ever felt was when I got an infection in my jaw after having my wisdom teeth extracted. Boy was that uncomfortable as hell.

The worst pain, eh? No question for me: When I was in full renal failure. When my kidney (I only got one to begin with) quit, I was slowly poisoning myself. I ran 104 fevers for 10 days before I couldn't handle it anymore and took myself to the ER. I was whisked into treatment immediately...apparently I was nearing death's door. My whole body was achy, and the whole universe irritated me. I will never suffer that in that capacity EVER AGAIN!

I got an infection in my colon which ruptured. The infection spread and I ended up with peritonitis. I had surgery to remove the damaged part of my colon and all of my internal organs rinced off. I have never ever experianced pain that bad. I have had 2 children one of which with no epidural. That infection was by far the most pain I ever had.

Got an ingrown toenail on my big toe when I was back in highschool. Instead of going to a podiatrist, my parents took me to a normal doctor.
The doctor "anesthesia'd" my foot, only he didn't. Came back a couple minutes later, my foot wasn't numb at all.
I told him, he didn't listen.
Also, the way he numbed up my foot, shot right underneath the toenail. Hurt like a bitch.
He then proceeded to take medical scissors cut my toenail in half, then rip out the ingrown side.
It was the equivalent to literal torture. I felt everything.

Hells yes. I had this happen in October and geez does it suck. They gave me some penicillin for the infection and my face blew up like crazy. I could feel my gums swelling in between my teef and it was just awful. The gums on the lower left of my mouth were all blown up like a balloon and the pain just shoots through your entire head. Just terrible.

Urm... Hm..
I keep getting these recurring, sharp, stabbing pains in my chest. I absolutely can't move when I get them because it intensifies like crazy. So I have to stay utterly still until it goes, which is pretty inconvenient when I'm walking around outside or something.
Sorta like, over to the left and quite deep. Used to get them in my right side, then before that they were kinda under the bottom of the right side of my rib cage.
Absolutely kills. Can't even breathe because of how much it moves my chest around. One time I had it whilst walking home from school, had to go to a friends house and lie on their bed for half an hour before I could breathe properly again.
So, theres the initial pain, then you can't breathe whole breaths which speeds up your heart rate which intensifies the pain even more. After a while it usually clears up.
I don't get it so much anymore, and I did go to my doctor once but he said not to worry... Which was.. comforting.
Apart from that I don't feel much pain... I'm numb.. all over...

Yeah that's pretty bad. Mine had to be broken apart and taken out piece by piece. I have no memory of the trip after he gave me the anesthesia needle, I remember the needle going in them waking up on my couch with cotton in my mouth. the day after was a living hell.

The worst pain I've ever been in is getting the top of my pinkie finger crushed in a door and nearly bleeding out from said injury. It was a combination of the chills and numbness from the lack of blood except my finger which hurt like a biatch. The tip was hanging off and was being held on by about a 1/4 inch of skin, my fingernail broke off and the dead flesh eventually fell off after two months with a new fingertip under it, which was extremely sesitive to pain for another month then another two months of physio before I could bend my finger properly again.

I had my right-side ring finger slammed in a door when I was 11. A very large, heavy door. A bruise took the nail clean off a few days later (More gross than painful, but I was 11, that part was cool), while I was left with a flap of skin on the opposite side. A fraction of an inch lower, I would have lost part of my finger. (And probably gotten a nice little settlement from the school) Hurt like a sonuva bitch. I uttered swears I didn't even know at the time.

Mine was gross as well as painful.
I had infected cyst on my lower back that grew to about the size of a half dollar (for those of you outside of the U.S. that's about the same size as a U.S. Half Dollar)...
Anyway, this red, throbbing, pus-filled growth needed to be "drained"... with scalpels, forceps, and NO pain relief of any kind.
I have never felt anything like it. Incredibly sharp, shooting pains along with agonizing aching pains, with a bit of squeezing and tearing thrown in for good measure...
I saw stars and nearly blacked out. The only thing keeping me conscious was my laughter. It was insane... though my post "draining" meds were wonderful!

Broke both bones in my lower leg falling on snow, my foot was actually pointing 90° degrees away in a direction it realy isnt supposed to be aible to point at all.
Laying in about 1 foot of snow and still snowing heavely I had to wait 45 Minutes for the ambulance to get there since the roads where backed up because of the damn snowfall...
By the time the got there i was cold as hell, and then they still had to straighten out and splint my leg before they could move me....OUTCH!
Not my best day.

It's a tie between when I fell off a semi truck in December of 2006 and broke my ankle (which I'm still coping with) and having a mole removed from my back between my shoulder blades.

It has to be either passing a kidney stone or when I slipped a disk in my back last year.
Well slipping the disk just felt like I pulled something. It was the pain I was in after.

A tie between kidney stones and the time I had both big toenails removed and when the aneasthetic wore of I found my toes had swollen inside the dressings and I had to chug painkillers until a few days later when they were finally replaced.
