The only ones I'm familiar with are...

For the Autobots: Superion (the Aerialbots) and Defensor (the Protectobots)

For the Decepticons: Devastator (the Constructicons), Menasor (the Stunticons), and Bruticus (the Combaticons)

Magnaboss, 3 into 1, (Repainted slightly for Japanese Release)
Tripredacus, 3 into 1, (Repainted slightly for Japanese Release as Tripledacus)

The RID Gestalts
Rail Racer, 3 into 1
Ruination, 5 into 1, (Bruticus repaint), (Repainted twice in series - Arctic and Desert Camo)

You forgot the Omega Prime combenation of Optimus Prime and Ulyra Magnus from RID.

And Armada, Energon and Cybertron all had Optimus combining with one or two othercharacters altho it seem that those that combined with him were just used as weapons or tools but in Japan they did name these different modes.

To be fair some of those are from the Japanese TF series after it left the air here in the US.

So in Fairness:
:BOT:
Defensor, Superion, and Computron.
:CON:
Devastator, Bruticus, Abominus, Menasor, and Predaking.

These were the only ones in the US run of cartoons.

The rest are from the Jap cartoons, that continued after the US run. In order; Headmasters, Masterforce, and Victory.

There was a single OVA called TF:Zone. The rest of Zone was carried in a Manga(and/or TV Magazine), and the last series of G1 TF's were in the Manga TF:Battlestars.

After that, everything was recycled.

The term Gestalt, is a fan word. It is explained in the wiki link that Dead Metal had: http://transformers.wikia.com/wiki/Combiner

Different culture, different ideas of what is "Cool". My little bro was fascinated by trains when he was a kid. But imagine a little Jap kid who see's a train zoom by at a 120 km an hour. I'm sure it's sight to behold.

The term as used in TF fandom is derived from a school of thought in cognitive psychology that, among other things, essentially suggest that the human brain will derive patterns out of otherwise unrelated elements. It's from the gestalt theory that we get the phrase "greater than the sum of its parts".
