I don't have it anymore because I lost it but I had a beast wars Dinobot where the he had two left legs or was it two right legs I don't remember. He wouldn't stand up properly because of that.

One factory mistake I can think of that is a bit of a problem is that when a toy is rereleased as a repaint or varient, the mold seems to have fallen apart to a certain degree.

For instance. The G1 Stepper reissue and also the Encore Meister all share the same flaws as the remade G2 Jazz mold. I'm not sure why the mold gets damaged as I have never seen the toy robot construction processes visually, but obviously something happened to the original mold that changed it before it was rereleased.

However, one factory mistake that I personally loved in the early days of Transformers, was that some boxes came with duplicate sticker sheets, instructions and/or catalogs. That was always a nifty mistake hehe!
