Have you ever read a Transformer comic or watched an episode, and like a particular character so much that you just wanted to buy the figure. Then when you get the figure, you feel extremely disapointed, and don't feel it accurately lived up to your expectations. This doesn't apply to characters who got no figures.
Conversely: Have you ever purchased a figure you thought was extremely awsome in one area or another, but felt that the character got jipped when they were put into the media? This does not apply to characters who got no media presentation.
Characters that didn't live up to the figure:
Energon Mirage: I have the Dreadwind repaint, but I think it's an awsome figure nontheless. However, Mirage was a total moron in the Energon show. Also, if I remeber right, he did some kind of weird dance when he transformed, and was not in any way the menacing Decepticon the figure made him out to be. Such a waste.
Energon Terrorcons: They got blown up WAY too much, and rarely offered a decent challenge to the Autobots. Did Insecticon or Cruelock ever even Transform?
Energon Starscream: One of my favorite Energon figures, practically reduced to a backround character. I don't even remember him doing a whole lot, or getting that many lines.
All the Armada Mini-cons in the show(save the Perceptor team, Starsaber, Skyboom shield, Requium Blaster): Beep beep, beep beep, beep beep. That tells me a lot. And they rarely ever showed up, or fought.
Energon Perceptor: Not necessarily the individual mini-cons, but rather the combined form. Did it appear any after the first episode?
Figures that didn't live up to the Character:
Uh...shoot, I can't think of any right this second.

Agree with all of those. For Figures that don't live up to the character, G1 Megatron springs to mind, and also I think Cybertron Lugnuts deserved something better, for some reason I like him.

Transmetal Rattrap. Rattrap himself said it best, "I feel naked without my gun!" It's a decent oy, it's just... There's so many ways it could have been better, and Rattap was just such a well-done character, that I felt kinda let down.
As for Characters that don't live up to the toy... I'll throw you the most obvious one of all that still qualifies. Nemesis Breaker. At least the Terrorcons got to swarm over stuff, all Nemesis Breaker did was follow Megatron around like a dog and occasionally become an arm. SO much wasted potential...

I've never owned Ironhide or Ratchet, but this one is so obvious it deserves an immediate acknowledgement. The toys were completely unworthy of these characters, and the animators did the smart thing by going with a completely new design.
Wheelie? What can I say about him? As annoying and pointless as the character was, he was a "regular" in G1's third season, so one felt obligated to get the toy for that reason alone. At least his cartoon design was slick looking. The toy was absolutely horrid. (I hesitate to say it's "unworthy" of the character, though!)
The Galvatron toy looked a lot different than the cartoon, but I've heard this was due to a last-minute change, so I'll go easy on this one.

Yeah, I've been limiting it to figures I have, but here's a couple I dont' have, but I feel the figure doesn't live up to the character:
Beast Wars Torca. NOT my observation, but it was someone else's, that I recalled. They said the figure was too small, while the comic portrayed it rightly as a humoungously awsome looking guy.
G1 Springer. The character does have a pretty cool design, but it translates very poorly into the figure, in my opinion. The car mode doesn't look very car-ry.

But, the toy existed first, so that's more of the comic writers/animators decided to improve on the toy instead.
Toys that don't cut it to character:
Devastator - Even compared to the other gestalts, this toy is so flimsy and erratic that it didn't work.
Unicron - Especially after seeing how they did Primus as well as they did, Unicron's toy is a joke.
Characters that don't cut it to toys:
Reflector - The toy was a kickin' exclusive, but they made him/them the drones of the Decepticons in G1.

Aside from the ones already mentioned, I think most of the gestalts are kind of disappointing. They're not nearly as big as they are in the cartoon... in the 'toon, Devatastor is gigantic, but toy Galvatron, G2 Megs, and even Blaster tower over him. Same goes for the rest of the G1 gestalts, maybe sans Predaking.

G1 Megatron. Good character, but the toy was rather gangly looking, and that crotch is laughable. Same goes for Shockwave. Dinobot....don't get me wrong, Dinobot's a nice toy, but too small compared to some of the other Maximals and Predacons.
G1 blurr toy was nice, but after the movie, he got reduced to a comedic role along with Grimlock.
