I wonder if Headmasters would count, since their heads are very visible.

I was looking at my Generations Brainstorm and wondering that myself a day or so ago. I guess it depends...
On one hand, when the head transforms into something else (such as Sky Lynx's cockpit) I wouldn't call that a visible head. And Headmasters' heads all do transform into little pilot figures.
On the other hand... most of those times, those pilot figures have a giant upside down face on their backs!
So... Six of one, half a dozen of another, I suppose!

Personally, I would have placed G1 Scourge as #1. I find it very clever. However Skystalker's visible head is pretty cool and it reinforces his name.

I tend to lead with my right, but always follow with a left. So here is my left.
One of my favorite and most unfortunate sufferers of said syndrome is Beast Machines, Basic class Quikstrike.

ROTF Ravage's head is visible in his "Vehicle mode" I would state.
Speaking of Ravage though I bought one at a table sale 2nd hand and was surprised to discover that there was a mould variation I was unaware of or simply had never noticed the one I had since 2009 has 1 less joint in the tail than the on I just bought which has an additional point of rotation above the second pin - I would guess this was a 1st run American design feature that they soon altered to less joints to prevent breakage.
But I was surprised as it was a variation I never noticed before and I have at least 6 Movie Ravage toys this was the 1st with that extra joint though.
