Is this on the PS2? I noticed a bug in Mercenaries where I had a flash grenade, used it, and kicked some guy's head off, but alas it didn't seem to count and my bonus in-a-row kept flashing and ran out.

In an case, a lot of games have adaptive AI. Enemies will actually read your actions, and find out better ways to kill you. In Metal Gear Solid, Psycho Mantis would tell you right out how you've been playing, how you'd been avoiding traps and taking out enemies.

I didn't have this much of a problem in Melee. Sure the bots were tough, but I think I was able to take out the needed number for some kind of trophy with Roy.
