This is a unique case, and I'll cut to the chase here:
I cant ask a doctor what's wrong because it's my friend, not me. And the real problem here is, he's really guarding what's wrong with him, playing it off like he's fine when he's really not. He wont go to the doctor's and wont talk about what's wrong with him, so I don't even now if you can help me with what limited info I have on his condition.
But if you're a doctor, please PM me, I need to talk to someone about this.

Honestly I think the best course of action is to remind your friend that you care for him and politely ask him to seek treatment. Don't push him too hard, but remind him that his health is important.
If you are a minor I think that going to your parents of school counselor might be an option if things are serious or get out of hand.

Physical or psychological symptoms?
Only way you can give a preliminary diagnosis is if you actually get the patient to give you an accurate medical history via interview.
Every effective medical intervention is based on an accurate patient history, not to mention diagnostic data like lab exams, Xrays/CT scans etc.
