I never found Ultimate OP all that complex. The cab part is pretty simplified compared to any of the other movie OPs, and the trailer is just a big armor to stick the cab in.
WFC OP is way more frustrating, though I don't know if I'd say it's the most so.

Those spring loaded legs won't cooperate in alt mode, robot mode, heck, the whole dang toy is the most frustrating waste of plastic in existence! No way that can be left off of any list. No friggin way.

I'm also going to RoTF Leader Jetfire as well as DoTM Leader Sentinel. And yeah, MP Rodimus is kind of a pain in the ass as well.
But I still love the way he looks in all 3 modes.

I think complex does not neccessarily mean annoying, and annoying doesn't neccessarily mean complex, as fenrir above mentioned about Tailgate.
I don't have that 51 step Prime you mentioned, but if those 51 steps are logical and intuitive, I have no problem, and would probably find it enjoyable. It's when those steps are either unclear, or (seem to) require partially undoing previous steps or rely on perfect placement of unsecurable parts, that I feel is annoying.
So you could probably make three lists here, "Simple but horrendously annoying," "Complex but relatively intuitive," "Complex and mind-destroyingly annoying"

Oh yes, that chest piece thing underneath doesn't really peg in anywhere, just kinda floats and wedges in there in vehicle mode.
Seconding the Classics Wreck-Gar mold. I dislike transforming cause of the legs/seat section you have to get JUST right, and the front wheel as well.
TM2 Megatron isn't too bad after awhile, but the wing backpack section kinda scrapes and that always annoyed me.
Classics Blitzwing cause of the pegs that don't lock in that well in either vehicle mode.
The Animated Voyager Seeker mold, another case where the legs have to be juuuust right for them to lock in.

Most annoying? Yes, please! ROTF jetfire deserves a massive drubbing! Like Noideaforaname, I loved Jetfire in ROTF. I looked forward to getting him in plastic form, but nope! A four-way loss...
1. Bad robot mode.
2. Horrid transformation.
3. SUCKTACULAR jet mode.
4. And his armor mode's not even all that great!
Not caring about scale since 1984. Just like Hasbro.

I do all my TF's without instructions, but there's only two I haven't been able to accomplish on my own: Alternators Skids and MP09 (Winnebago mode).

As far as annoying goes RTS Perceptor is up there, I always have a rough time getting him all pieced together in vehicle mode. Then there is TFCC Rewind and Eject, those piledrivers keep popping out when trying to get them in tank mode all lined up. But I love these figures none the less.

Haven't got those two, but it's the same for United Rumble & Frenzy - either those springs are too strong, or the tab holding them in is just a little too short.

Good lord, I will never understand why so many people have such a hard time with that mold. For me, he's very intuitive and fun to transform. Kinda baffling, really.
One very annoying is the Alternity Seeker mold. Loose everything, the doors that keep popping off, the failed auto-morph, and the fact that the chest just wont peg in the crotch part. I have Starscream but why the hell did I impulse buy Bludgeon?
At first, I had a hard time and I hated ROTF Leader Optimus. Not anymore. Still, transforming him take waaaaaayyyyyyy too long so he's foreverabot.
Ultimate Optimus Prime? Just the step of figuring out how to fold in the armor arms in the trailer. Same thing with Alternators Grimlock/Wheeljack.
MP Rodimus. Not hard to do, but so not fun that I sold him without regret.
My #1 pick: I really hate transforming that type of TF. The G1 Scramble City Combiners. Vehicles are good, but the bot mode are so underwhelming, it's pretty sad. The only mode that is really worth it is the Ghestalt mode and it require so many little parts that are useless for the other modes. Plus don't lose any bits!

ROTF Leader Optimus was frustrating at first for me, although not for the commonly-cited clearance issues. Rather, it was the counterintuitive step of disconnecting his chest halves from where they very tightly peg into the torso. Now, though, I transform him for fun.
I'm surprised I haven't seen Cybertron Thundercracker here, considering how much I've seen him (unfairly) blasted for this. Have people finally learned that his left shoulder's strut is supposed to be pushed in between his chest and his back, rather than (as the stupid instructions say) rely on the hinge to stay upright?
Of what I have, it's actually a fairly simple figure who can be annoying: G1 Megatron. He's a lot more accurate than he gets credit for, but those die-cast thighs.. The upper lock on his left calf periodically wears loose, and lets his leg collapse when I try to stand him in robot mode. That is very irritating, let me tell you. It wouldn't be a problem if his thighs were plastic, but since they're 5mm thick slabs of die-cast they make him top-heavy.

Maybe your G1 Megs is just old and used a lot. This could happen with 30yo toys.
I recently got my G1 Megs re-issue. It was like brand new and I had it at a relatively good price. I really LOVE trnasforming him! It's fun, intuitive, and he's TIGHT all over. The only annoying part is the "arm rest" accessory of the gun. It's just a couple of bits that peg inside one another and the whole assembly can pop off pretty easely when I want to go PEW PEW PEW! Other than that, I reccommend G1 Megs to everyone. Sure, he look like a derpy goblin. But he's a cute and fun goblin!

Given the stress point in the gun handles/thigh, making it plastic would make it prone to breakage. I have a simple fix. Unscrew the red panel and add a dab of nail polish on the pegs that locks on to the die-cast thighs. Not a permanent fix unless you keep on transforming him. Same issue with G1 Astrotrain, the legs get loose over time.

Oh, he's definitely old and I've tightened the lock before - he just wears the tightening out faster.
